Planning ahead "Informational Only" or can be "Pre-Paid".
Planning ahead "Informational Only" can be done at no expense, and consists of recording one's preferences for:
- Vital Statistic Information
- Service Preference (Viewing, Memorial or Graveside Service)
- Casket or Cremation Urn
- Final Disposition (Burial, Entombment or Cremation)
- Burial, Entombment or Cremation Vault
- Church, Chapel or Synagogue
- Final Resting Place (Cemetary or Mausoleum)
- Prayer and Acknowledgement Cards with your preference of prayer or expression
- Floral Remembrances
- Memorial Contributions
Planning ahead for "Pre-Paid" services, consists of the above information and the placement of financial resources to be used at the time of death. These resources can be placed in a funeraltrust account or life insurance policy.
Death Certificate
A death certificate is the permanent record of death which is filed with the Bureau of Vital Statistics,
or the Town Clerk's office in the town in which the death occurred. A death certificate is required to
obtain a permit for burial, cremation or removal. Certified copies of the death certificate are required
for insurance claims, banking institutions, stocks, bonds, etc. As part of our service, we will assist you
in determining how many death certificates you will need, and can usually provide the copies to you on the
day of the funeral. For deaths in the State of Connecticut, the fee for each certified copy is $5.00,
however, the fee varies from state to state. Our dedication to the families we serve does not end with the
funeral. We're here for you . . . Should you discover at a later time that you need additional copies,
please feel free to call us. We would be glad to assist you in obtaining them, or you may contact the Bureau of Vital Statistics.

Social Security
When a death occurs, our staff provides a Statement of Death, notifying the Social Security Administration.
However, an application for benefits must be made by telephone, or by going to any Social Security office.
If the deceased was receiving Social Security benefits, any checks which arrive after the death has occurred will need to be
returned to the Social Security office. If checks were being directly deposited into a bank account, the
bank needs to be notified of the death too. The current death benefit is $255, payable only to a surviving
spouse or dependent children. In some instances, the surviving spouse's monthly benefit may increase.
If there are dependent children under the age of 18, or over 18 and enrolled full-time in school, there
are usually monthly benefits paid to these children.

Vehicle Transfer
In order for a vehicle to be transferred to you from a deceased immediate family member you must
proceed to your local DMV full service branch office. Section 6 of the H-13 Application for Registration
and Certificate of Title must be completed for a transfer from the estate to a member of the immediate family.

Most life insurance companies require a certified copy of the death certificate be submitted with the claim
